Short-Term Rehabilitation
At WestLake Care Community, we collaborate with our short-term rehabilitation Care Partners to develop the tools and support needed in order to reach their rehabilitative goals for returning home. Care Partners will work with a comprehensive team of professionals who will help reach their goals through a person-directed plan of care.
Our team strives to be successful in facilitating our rehab Care Partners to identify and to meet their rehabilitative goals. Our rehabilitation program focuses on building relationships that promote participation in individualized therapy programs and reaching goals as soon as possible.
