Must be full-time (work average of 32 hours or more per week).

Eligible the 1st of the month following 90 days of service for Health Insurance, first of the month following 30 days for Flexible Spending.

If an employee declines coverage or fails to sign up during their eligibility period they will have to wait until annual enrollment (April 1).

If the individual experiences a “qualifying change of events”:

  • Spouse loses coverage
  •  Birth or adoption
  • Marriage/Divorce
  • Legal Guardianship,

the employee will not have to wait for annual enrollment to be added to the plan.

Dental/Life/Disability/Cancer/Accident Insurance (Voluntary Benefits)

Must be part-time (work average of 20 hours or more per week).

Eligible the 1st of the month following 30 days of service.

If the employee declines coverage or fails to sign up during their eligibility period they will have to wait until annual enrollment (April).

401K

Employees who have twelve (12) months service, worked 1000 hours in the previous year and are 21 or older are eligible to participate. Quarterly enrollments only January 1, April 1, July 1, Oct. 1

Discount Bus Passes

Monthly bus passes are available for employees and can be purchased using payroll deductions.

Meals

Meals are available in the Dining Room for a small fee. Employees must sign up for the meal before the meal is served. The sign-up sheet is located in the dining room.

Vacation Pay

Must be full-time for 1 year (working at least 32 hours or more per week)
The number of vacation days that a full-time non-exempt or exempt employee is entitled to each year is determined from the employee’s length of service at his/her anniversary date.

For example:
The following schedule is base on a non-exempt employee:

  • 1 year of service = 5 days (40 hours)
  • 2-4 years of service = 10 days (80 hours)
  • 5+ years of service = 15 days (120 hours)

For example:
The following schedule is based on an exempt employee (includes LPNs and RNs):

  • 1-4 years of service = 10 days (80 hours)
  • 5+ years of service = 15 days (120 hours)

Holiday Pay

  • New Year’s Day
  • Memorial Day
  • Independence Day
  • Labor Day
  • Thanksgiving Day
  • Christmas Day

Hourly employees who work on these days will be paid time and one-half for the shift beginning at 10:00/10:30 PM on the eve of the holiday.

Salaried employees who work on holidays will not be paid time and one-half.

Holiday pay is not included in total hours worked for the purpose of calculating overtime.

Tuition Reimbursement

Full time employees who have completed at least 1 year of service or more and are in good standing with the company (no written warnings present in personnel file) are eligible to participate in the educational assistance program. When an educational course is job related and will assist in improving job efficiency and performance, employees may request (fill out Educational Assistance Application), in advance each semester, that the course be considered for reimbursement. In addition to the EA application a course description must also be attached. All courses must be pre-approved by the department manager and Administrator. Reimbursement is limited to $650.00 per semester and is based on individual performance in the class.

  • For every A that is earned: Course is fully reimbursed
  • For every B that is earned: Course is reimbursed at 70%
  • For every C that is earned: Course is reimbursed at 40%
  • For D’s and F’s No reimbursement

Referral Bonus

All hourly associates are eligible to participate! By referring an LPN or RN you are eligible to receive a bonus on your paycheck in the amounts
Below, if the RN or LPN is hired:

Full Time Employee Referrals:

  • $50 – When the referred employee starts
  • $50 – Each time at 30, 60, 90 and 180 days

Part Time Employee Referrals:

  • $50 – When referred employee starts
  • $50 – When referred employee hits 90 days
